This test requires a lead requisition form (see link above). Please fill out the form COMPLETELY. Venous specimen preferred. Recommend tan top plastic tube certified lead free (Becton-Dickinson). Capillary specimen also acceptable if patient's hands are carefully washed BEFORE collection. Lavender (EDTA) and royal blue top tubes with EDTA additives may also be used.
